A 40s F breast clinic pt is sent to the ED because of a rash. She had been on keflex / bactrim 2 weeks ago for a breast abscess that developed after a biopsy. The pt had a repeat aspiration two days ago and was re-started on bactrim. The rash is on her legs. It had popped up a week ago, and now more lesions are coming up in the past two days. They are more itchy than tender. The lesions are mildly bumpy, but not tender. What is the dx?
Hard to make out the rash on the pic.
Is it two big red nodes? Urticaria? Or is it a bunch of tiny bumps with two bruiser looking things?
Top of head… Drug reaction given timing and widening with redosing.
Obviously, crazy cancer stuff pops into mind too. Inflammatory breast CA that is showing via lymphatics after biopsy?
Either way back to cancer doc and Derm. Stop antibiotics. Symptomatic treatment. Could even Rx epi pen
is it raised/nodular? erythema nodosm vs urticaria?
